The SprintChoice provider network for the Kansas City area includes thirteen (13)
counties. If you live in one of these counties, this network applies to you and your covered
dependents enrolled in SprintChoice.

The online directory lists all categories of providers who participate in the Kansas City SprintChoice
network. You may search for providers by provider type, location or provider name.
The results of your search are sorted alphabetically based on your search criteria.

Summary Plan Description (SPD).

Participating providers are subject to change. Please confirm provider participation before
you receive medical services. To determine the current status of a provider,
call Fiserv Health at 1-800-288-5366, and contact your
provider directly and ask if he/she is participating in the SprintChoice provider network.
